Care Manager

SeniorCare is looking for a dedicated Care Manager to join our team on a full-time basis. In this role, you will play a crucial part in providing consumer-centered assessments and facilitating the authorization, coordination, and monitoring of services for elderly individuals, enabling them to lead independent lives within their community.

Qualifications:

  • Bachelor’s degree in social work or a related field is required.
  • Strong communication skills are essential for effectively engaging with consumers, their families, and support networks.
  • Proficiency in computer usage, including data entry, word processing, and research, is necessary.
  • Prior experience working with elders and/or in a community service setting is highly desirable.

Clinical Assessment and Eligibility RN

SeniorCare has an opening for a Clinical Assessment and Eligibility RN. This position will be scheduled on weekdays during office hours with no holiday, night or weekend hours required! The CAE RN assesses the eligibility of community residents for a variety of community based programs. It requires an RN with a MA license, intermediate level computer skills and two years of clinical experience. Hands-on skilled nursing services are not part of this position, however excellent organizational and assessment skills, as well as the ability to independently manage a caseload are critical. Care Manager Administrative Assistant

SeniorCare is seeking a dedicated and compassionate individual to join our team as a Care Manager Administrative Assistant (CMA). This pivotal role supports our care management team by executing various administrative tasks crucial for departmental efficiency. The Care Manager Administrative Assistant (CMA) plays a vital role in our care management team, communicating with consumers, managing documentation, and providing essential administrative support. Responsibilities include communicating with consumers as directed by the care management team to confirm their continued need for services and explore additional support options. Additionally, the CMA assists care managers with check-in calls to consumers to confirm service satisfaction and assess ongoing needs and serves as backup for care manager on duty calls.

Document management is a key aspect of the role, maintaining accurate records of all communications with consumers, service providers, and other contacts. The CMA also monitors expiring care plans, reviews daily admission/discharge information to track consumer transitions and ensures closure of all appropriate cases monthly.

Qualifications for this position include a high school diploma or equivalent, exceptional communication skills, strong organizational abilities, and proficiency in computer applications. A compassionate attitude towards consumers and a commitment to confidentiality are essential. Per Diem

Clinical Assessment and Eligibility Registered Nurse

SeniorCare is seeking a dedicated and experienced Clinical Assessment and Eligibility Registered Nurse (CAE RN) to join our team. This per diem position offers flexibility and autonomy, allowing you to conduct in-home assessments in the Cape Ann area according to your schedule. With no office hours required except to bring in assessment forms and pick up assignment tasks, you’ll have the freedom to manage your workload efficiently. As a CAE RN with SeniorCare Inc., you’ll play a crucial role in assessing the eligibility of community residents for various community-based programs. We value organization, confidentiality, and open communication, making these traits essential for success in this role. Your reliable transportation and legible handwriting will ensure seamless coordination and documentation of assessments. While hands-on skilled nursing services are not part of this position, your intermediate level computer skills and two years of clinical experience, coupled with excellent organizational and assessment skills, will empower you to independently manage a caseload effectively.  (updated  4/3/2024)

AAA/Community Programs Administrative Assistant (20 hours per week)

SeniorCare is seeking a part-time Administrative Assistant to support SeniorCare’s Area Agency on Aging and Community Programs. Responsibilities include administrative support, managing program statistics, assisting volunteers, and supporting various Community Programs.
